Admission details recorded include the date of admission; name and address of patient; age, sex, religion and marital status of patient; occupation (listed in the 'If born in hospital, name of mother' column); name and address of nearest relative; the amount of days in any given month spent in the hospital; and the date of discharge. Discharge columns also record if death occured during hospital treatment.
