Period in institution: Date of discharge or death, no of days for month, number of days for the year.

Insert 'Died' in case of death in hospital (used to record transfer to hospital, death, other institution, boarded out).

While the template for these registers is designed for hospitals, it is used for the county home combined register and statistical record, with the medical columns such as infectious disease classification left blank. The column for 'Disease or Sickness' is used to record the reason for admission such as 'Destitute'. 'Mat.' (maternity), T.B., 'Waiting Case' (late pregnancy), 'Born'., etc.

Alphabetical index of surnames precedes each register.
